Ruby’s Heart Home Intentional Shared Living Program, LLC accepts referrals from community partners, agencies, and individuals who believe a resident would benefit from a structured, peaceful, adult‑centered shared‑living environment.

The Referral Form is available for agencies and professional partners to review and complete. Please ensure all sections are filled out accurately to avoid delays in the screening process.

⭐ Referral Form Submission
Completed Referral Forms and supporting documents may be submitted electronically. Referral partners may send forms directly to our program email or upload them through the designated submission option on our website.
Please ensure all required documents are included before submitting.
⭐ After You Submit
Once your referral is received, our team will review the information, follow up with any needed details, and begin the screening process. Decisions are typically made within 24–48 hours.
